Location:
We are located in what was formally the stretching room at the back of the fitness centre. It is a private room seperate from the exercise facilities.
Since we are located in the fitness centre all WestJet employees wishing to make use of our services will be required to complete the online fitness centre orientation.

Availability;
Massage: Monday & Wednesday 10:30 am – 4:30 pm
Chiropractic: Tuesday & Thursday 11:00 am – 3:00 pm
Acupuncture: Tuesday 3:30 pm - 7:00 pm
Naturopathy: Friday 10:30 am - 4:30 pm
Fees:
The fees for services administered onsite at the Calgary campus will be at a 10% reduced rate when compared to our clinic fees as outlined under the “Fee Schedule” tab.
This reduced rate will also apply to WestJetters and their immediate family members at our Kensington clinic location as well but appointments for our Kensington location must be booked by phone at this time.
NOTE: The initial fee for chiropractic will be reduced to a subsequent visit fee if you have seen a chiropractor in the past and are wishing to continue care at the campus location for convenience.
Payment For Services:
· The payment for services are due when services are rendered.
· Preferred form of payment is by Credit Card and Interac but Cash and Cheques will also be accepted.
· Receipts will be issued and may be submitted for reimbursement to the insurance company for possible reimbursement.

When scheduling your first visit please keep the following in mind:
· Remember to download, print, and complete the necessary forms below

· KHG Clinic Intake Form (Needed for All First Visits to the clinic but only filled out once)
· along with the form relevant to your selected appointment type:
· KHG Chiropractic Intake Form (Needed for First Chiropractic Visit)

· If an appointment type is not showing in the menu as a choice there is not enough time available for that type of appointment
· When scheduling an initial chiropractic appointment
· If you have never seen a chiropractor prior to this visit choose “First Visit – No Prior Chiro”(30 Min, $81.00)
· If you have seen a chiropractor prior to this visit choose “First Visit – Prior Chiro Patient”(30 Min, $40.50)
· If you have seen Dr. Sembrat prior and for all future appointments choose “Chiro Treatment”(15 Min, $40.50)
· When scheduling a Custom Fitted Foot Orthotic Assessment
· We do not charge for the assessment itself
· The only charge occurs if the orthotics are ordered
